Shoulder Pain after Sports in Active Adults 2026

Hey everyone! As an orthopaedic surgeon in KL, I see this all the time — active young adults who dismiss shoulder pain as “just muscle soreness” after badminton, futsal, or gym, only to find months later it has gotten much worse.

Let me break down rotator cuff injuries — what they are, how to spot them, and what you can do before seeing a doctor.

What is the rotator cuff?

It is a group of 4 muscles and tendons that keep your shoulder stable and allow you to lift, throw, and rotate your arm. When these tendons get overstressed or torn — from repetitive overhead motion, a fall, or a sudden jerk — you have a rotator cuff injury.

Signs to watch out for

  • Pain with overhead reach
  • Hurts when smashing, serving, or lifting above shoulder level
  • Night pain

Wakes you up when you roll onto that shoulder in bed

  • Weakness

Arm feels heavy or lacks power during the smash or throw

  • Clicking or catching

A clunk or grating sensation when moving the shoulder

  • Painful arc

A specific range — roughly between 60° and 120° — where lifting your arm sideways hurts the most. Below and above that range, it’s fine. Classic sign.

What you can do at home (first 48–72 hours)

1) Rest and modify activity

stop overhead sports for now. You don’t need to stop moving entirely, just avoid the painful range.

2) Ice it

15 to 20 minutes, wrapped in a cloth, 3 to 4 times a day. Don’t apply ice directly on skin.

3) Anti-inflammatories

over-the-counter NSAIDs like ibuprofen can help with swelling and pain. Take with food. Max 3–5 days without doctor advice.

4) Gentle pendulum exercises

learn forward, let your arm hang, and make small slow circles. Keeps the joint from stiffening up.

5) Posture check

slouching forward (common with phone/laptop use) increases shoulder stress. Sit upright, shoulders back and relaxed.

Please see a doctor if you have any of these

: Pain lasting more than 2 weeks despite rest
Significant weakness

: unable to lift arm at all
You felt or heard a “pop” at the moment of injury
Night pain that is consistently disrupting your sleep
Shoulder looks visibly deformed or drooped

: No improvement or worsening after home care
Most rotator cuff injuries are treatable without surgery — especially when caught early.

: Physiotherapy and a proper rehab plan gets the majority of my patients back on the court. Don’t wait until you need an MRI to find a massive tear.

Your shoulder is not just “getting old” — it is telling you something. Listen to it.
