Pre-Total Knee Replacement patient information in 2023 by Dr Sanjiv Rampal, Orthopaedic Specialist, Petaling Jaya.

Total knee replacement is a relatively common, safe and good option for chronic osteoarthritis knee . Osteoarthritis (OA) is the most common joint disease, affecting an estimated more than 240 million people worldwide, including an estimated more than 32 million in the US. Osteoarthritis is the most frequent reason for activity limitation in adults.
This operation is usually well-planned and will allow patients to be back at home in matter of days. The following are common questions and advice for patient doing the procedure in Malaysian hospital setting

What do I have to prepare for the operation?

The best preparation for the operation is adhering to the doctor’s advice. I would suggest continuing all current medications except for blood thinners or aspirin. Pre-operative optimization of your present underlying diseases like diabetes mellitus and hypertension would have been sorted out by pre anesthesiologist clinic visit.

Below are few points of consideration when preparing for the total knee replacement surgery.

  1. Getting used to assistive devices of ambulation is encouraged. Size and exact walking frame would be ideal. I would advise my patients to visit a local pharmacy or rehab center to find a suitable frame size 1 week before.

2. Taking all currently prescribed medications

3. Stop all supplements including common ones like glucosamine, choroiditin and turmeric powder

4. Not to apply ointments, creams or specific agents especially steroidal creams over the knee

5. Have your allergy card available

6. Have pre-operative dental and podiatrist check up

7. Inform surgical team if unwell,fever-like symptoms or flu-like symptoms

8. Make arrangements for a relative or family member to accompany you to and from hospital.

9.Make transport arrangements which accommodate wheelchair mobilisation

10.Practice immediate post operative limb exercise eg ankle pump, straight leg exercise, quadriceps exercise

Lastly in local settings patients are usually admitted earlier to the hospital and discharged at day 2-3 post operation with knee care advice and simple dressing bandage. Patients are encouraged to walk with walker after surgery .
